Event Name: Mondello Masters Superbike Championship       

Declaration by all Signatories I declare that:

I have been given the opportunity to read Motorcycling Ireland's Standing Regulations, the Dunlop Masters Superbike Championship Sporting Regulations and the Supplementary Regulations for this Event and agree to be bound by them. I declare that I am physically and mentally fit to take part in the Event and I am competent to do so. I acknowledge the potential risks inherent with motor sport and accept that these risks may give rise to my suffering personal injury or other loss.

I am not currently experiencing any symptoms of COVID-19 and have not been in contact with anyone showing symptoms within the last 14 days, If after submitting this form I do come into contact with someone with COVID-19 or if I start to exhibit any of the signs indicating that I may be infected I will immediately withdraw from the Event, notify the club running the event and ensure that my close contacts also do not attend. Should I become ill at or start to exhibit COVID-19 symptoms at the event I shall withdraw safely and notify the Secretary of the Meeting by telephone / SMS accordingly including identification of those others who I have come into contact with at the event.

I the undersigned herewith declare that the above details are factually correct and true. I furthermore confirm that I fully indemnify Motorcycling Ireland and Mondello Park in respect of any injury, loss or damage that I may suffer as a result of being exposed to any other competitor, official, marshal or event attendee in relation to the Covid 19 Infection/Virus and Covid 19 Pandemic generally and furthermore fully indemnify Motorcycling Ireland and Mondello Park in respect of any injury, loss or damage that may occur to any other competitor, official, marshal or event attendee as a result of being exposed to me.

I agree to abide by all Government, HSE and Motorcycling Ireland and Mondello Park requirements imposed in respect of COVID-19. I understand that a breach of my obligations within this document may lead to being disqualified from the Event and heavy additional penalties.

To the best of my belief I or the rider(s) who I am representing possess(es) the standard of competence necessary for the event to which this entry relates and that the vehicle(s) entered conform(s) to the regulations of the event.

I agree that should I at the time of this Event be suffering from any disability whether permanent or temporary which is likely to affect prejudicially my normal control of the vehicle, I may not take part unless I have declared such disability to Motorcycling Ireland which has, following such declaration, issued a licence which permits me to do so.

If I am the Parent/Guardian/Guarantor of the rider I have countersigned the competitor licence application for, I understand that I shall have the right to be present during any procedure being carried out under the event supplementary regulations and the General Regulations and that I have acquainted myself and the minor with them, I agree to pay any appropriate charges and fees pursuant to those Regulations and agree to be bound by and submit myself without reserve to the consequences resulting from them (and any alteration thereof). Further, I agree to pay any liquidated damages any fines imposed upon me up to the maxima set out in the event guidelines.

Note: Where the Parent/Guardian/Guarantor will not be present the Parent/Guardian/Guarantor must advise the organisers in advance, in writing, authorising their appointed representative to so act.

I hereby agree to abide by all applicable Motorcycling Ireland and Mondello Park Policies and Guidelines including, but not exclusively, Covid-19 restrictions and requirements,Anti Alcohol and Drugs policies and the National Sporting Code of Conduct.

I have read and shall respect the regulations for Control of Drugs and Alcohol as contained in the Standing Regulations and the event supplementary regulations and have also fully familiarised myself with the information on the Motorcycling Ireland web site, in particular the Sport Ireland Anti-Doping Rules which have been adopted by Motorcycling Ireland. Further, if I am counter signing as the parent or Guardian of a minor then in addition to the deemed consent to the testing of that minor. I hereby confirm that I give such consent for the minor concerned to be so tested.

I understand and agree that my personal data is being processed solely for the purposes of running this Event and may be used for the purposes of COVID-19 infection tracing, and will be handled by the organisers in accordance with data protection legislation.

I confirm I will bring all required documentation, licences etc., to the event as usual and I understand that spot checks will be made with regard to documentation and technical checks.

RISK WARNING, DISCLAIMER, AND INDEMNITY

1. I confirm the contents of my licence application and entry form are true.

2. I will satisfy myself (by sighting lap or otherwise) before taking part that the venue and track is acceptable to me with regard to its features and physical layout (unless prohibited to do so).
3. By taking part I accept the risks involved including the risk of injury or death.
4. I declare that I am competent to take part.
5. I declare that my vehicle is safe, complies with the regulations and is fit to use in this competition.
6. I am not taking any drugs (prescribed or otherwise) that will impair my ability to take part.
7. I have read, understood and will comply with all regulations and instructions relevant for this event.
8. If under the age of 18, my parent / guardian has read the above and signed opposite my name to confirm agreement with the declaration.

Furthermore, in exchange for being able to attend or participate in the event, I agree to release Motorcycling Ireland and Mondello Park from all liability for:

a) my death.

b) any physical or mental injury (including the aggravation, acceleration or recurrence of such an injury);

c) the contraction, aggravation, or acceleration of a disease.

the coming into existence, the aggravation, acceleration or recurrence of any other condition, circumstance, occurrence, activity, form of behaviour, course of conduct or state of affairs that is or may be harmful or disadvantageous to me or the community or that may result in harm or disadvantage to me or the community, howsoever arising from my participation in or attendance at the Event; to indemnify and hold harmless and keep indemnified Motorcycling Ireland and Mondello Park to the maximum extent permitted by law in respect of any Claim by any person; and to attend at or participate in the Event at my own risk entirely.
